Thank you for participating in our survey!
Please read all of the Navigation Notes before proceeding into the survey. When you are ready to proceed please enter your PIN (as found on your invitation letter), then click the Login button to begin.
Please use the NEXT and EXIT buttons on each screen. Do not use your browser's Back and Forward buttons. If you do, you'll be locked out for a short time and need to re-enter your PIN to complete the survey questions.
The survey will not allow you to go back to a previous question once you click on the "Next" button, so consider your answers carefully before going to the next question.
As you progress through the survey each time you press "next" your answers for that question will be saved. If your survey remains inactive for 30 minutes or you lose your internet connection your survey session may end. If your session ends you will be able to return to the survey and continue where you left off by accessing the link in your letter and entering your PIN again. If you need to leave the survey at any time please use the "exit" button and select "I have no time right now but will return later". Later, simply return to the address in your letter to resume the survey.
While every effort has been made to test our survey in many different browsers we cannot guarantee that the survey will function properly in older browsers. We recommend that you use one of the following browsers: Internet Explorer version 8 or higher; Mozilla Firefox version 10 or higher; Safari version 5 or higher; or Google Chrome. If you experience any problems with the survey we recommend trying a different web browser before contacting support.
